Medical Clearance Instructions for Returning Student Athletes
Please make sure the following forms are completed and in our possession by July 31
The following forms need to be taken with you to the Medical Doctor / Nurse Practitioner / Physician Assistant for your physical, with part one (1) completed before you see the provider. The provider should do a thorough review of your comprehensive health history and pre-participation medical questionnaire, and then perform the physical exam. Please check each form to ensure that the provider has completed everything, including phone numbers so that we may call and verify the date of your exam. The Comprehensive Medical History (Part 1) and Physical Exams (Part 2) forms are available to download and print back on the Athletic Training webpage.
Comprehensive Medical History (Part 1) – This form is divided by body part to give us an accurate accounting of where you are physically and any history that could impact your participation in athletics. Try and give us as much information as you can, including any physician’s names and phone numbers, so that we might be able to access these notes if you have a re-injury here.
Physical Exam (Part 2) – The provider should be very thorough with this exam, and check each segment of your body. If there are any issues that you feel need to be brought to his or her attention, do it as that body part is being evaluated. Please make sure that the provider has signed and reviewed all the forms before you leave the office.
Online Portal Registration Process
In order to access the online portal, go to snu2.atusers.com. Your Athlete ID will be your first initial, followed by your last name. For example, if you name was Lacy Smith, your Athlete ID would be Lsmith. Your first time sign on password will be “Snu”. Once you sign on for the first time, you may be prompted to create a new unique password, and it is important that you do so.
General Information—Please fill out all blank information that is applicable to you here, and verify that it is correct.
Medical History – Within this tab, at the top there is a section to add any type of surgery that you have had over your lifetime. Please be very specific with surgeries that are directly related to your participation in sport or have an impact on your ability to participate in athletics.
Insurance – To complete the insurance page, click on Add new insurance company and input all of the required fields. If you have questions please make sure to talk with your parents/guardian to complete this section. If you have NO INSURANCE, click the No insurance box at the top of the page. If you have questions please contact the sports medicine staff and we can help direct you to a resource to find coverage, and do not forget to save these changes at the bottom of the page.
Emergency Contacts – Please add contact information for all of the required fields on this section. Include phone numbers, email addresses and mailing addresses for each contact listed. We cannot stress enough the importance of completing this section accurately and completely and saving it at the bottom of the page.
Athlete forms – There will be 8 (eight) tabs listed in the drop down menu that will be need to be completed by a new student athlete. Please complete the following forms:

- Drug Testing Consent
- Medical History New and Transfer Athletes Only
- PHI Disclosure/Consent
- Policy Acknowledgements
- Securing Medical Assistance Policy
- Sickle Cell Trait Waiver
Please make sure to save each form once completed. If these are not saved they will have to be completed again. These will need to be submitted prior to the July 31 date for review, the sports medicine staff will let your coach know what if anything needs to be completed / resubmitted.
e-Files – Physical Form: Here is where you will upload your completed and signed physical form. You can scan it to your computer, or take a picture of it with your phone and email it to yourself, then save it to your computer that way. If you upload a picture of it, please make sure the entire paper is clear and visible.
If you have an DVD's' of an MRI or X-rays, these can be placed in your e-file. Follow the directions and make sure to save each time you add an item.
